Cancelled Appointments
Appointments that are not kept, or that are cancelled without 24 hours
notice, are a loss for everyone. Please inform us at least 24 hours in advance of any appointments you will be unable to keep to avoid any broken appointment fees.
Dual Insurance
For persons with two dental insurances, we will honor your primary insurance, and submit claims and necessary information for you. The patient is responsible for all secondary insurance.
Returned Checks
There is a $25.00 fee for all returned checks.
Six Month Appointments
We attempt to schedule six month cleaning appointments in advance. When you visit for your new patient exam and cleaning, please be prepared to schedule a six month appointment.
We provide courtesy confirmation calls prior to all appointments. If we are unable to reach you, we try to leave a message, whether at home or work, with a person or by voice mail.
If you prefer to be notified by e-mail concerning an up-coming appointment, please let us know by e-mail today.
Although attempted contact is made prior to each scheduled appointment, the patient is responsible for all missed appointments.
